Observation

During the inspection, Licensor Harris observed plastic and metal poles in the outside play area that are accessible to children. Licensor Harris observed a broken wood fence. The wood panels were placed in a dishelved manner. Licensor Harris observed a broken gate with exposed bolts and broken handle in the kitchen.(3)(a) During the inspection, Licensor Harris observed a broken toilet seat in the bathroom. A large piece of the toilet seat brim was broken. Licensor Harris observed an old infant stoller, DirecTV satellite dish and old toys in the outside area accessible to children. Provider mentioned that these are old toys and materials children do not use. (3)(g) During the inspection, Licensor Harris observed an outlet in the bathroom that was not tamper-resistant and GFCI outlet type. (5)(b)
